Comparison of Plate Osteosynthesis and Tension Band Technic in Simple and Comminucated Olecranon Fractures

Objective:

In this study we try to eveluate the results of tension band osteosynthesis versus plate osteosynthesis in olecranon fractures.

Material and Method:

We analyse 58 patients with olecranon (Mayo tip 2A ve 2B) fractures operated in our clinic between Januvary 2012 to December 2013 retrospectively. All patients were evaluated for active range of motion, pain scores with VAS, and for daily activity with Mayo Elbow Performance Score (MEPS). True anteroposterior and lateral radiographs were obtained. Complications and resurgeries were recorded.

Results:

Mean follow up was 20,6 months (minimum 12 to maximum 36). In plate osteosynthesis group mean VAS score was 1.9 (0-5), MEPS was 87.3 (65-100). In tension band group mean VAS score was 1.6 (0-4), MEPS was 92.6 (75-100). While mean MEPS scores were good in plate osteosynthesis group, were perfect in tension band group. Minimal loss in range of motion and physical performance were seen in both groups but this was not significantly important (p>0.05). No nonunion was seen in both groups. In 6 patients, 4 (14.2%) were in plate osteosynthesis group and in 2 (6.6%) were in tension band gruop, minimal degenerative changes were seen.

Conclusion:

As a conclusion no clinically significant difference between tension band technic and plate osteosynthesis in olecranon fractures was seen in our study. Surgeon could choose both technics according to his or her clinical experience.
