Infections in Combat / Terrorist Injuries and Rational Antibiotic Use

Nowadays, the war and terrorist injuries have a high probability of complications due to complex and multiple injuries. One of the most important complications is wound infections. Infections associated with combat-related injuries have been a problem that has been going on for centuries, because of the lack of clinical studies and limited data on this topic. All wounds incurred on the battlefield are grossly contaminated with bacteria, and can cause different infections in the process. The causative microorganisms are variable due to the geographical conditions at which the injury occurred, the characteristics of the terrain, climatic conditions, agricultural areas or city settlement. Although wound infections are preventable, mortality and morbidity of wound infections are increasing worldwide due to lack of clearly defined measures. Prompt surgical debridement and wound irrigation are the cornerstones of prevention of wound infections, and prophylactic antibiotic usage plays a complementary role. In this article, identification of infections due to war / terrorist injuries, distribution of causative microorganisms, risk factors, prevention and treatment approaches are examined. The importance of rational antibiotic usage at prophylaxis and treatment has been discussed.
